Service Facilitation
Home-Based Services
A Service Facilitator for children and adults living at home with family: case management, Waiver and Medicaid State Plan access, and day-to-day oversight of the service plan.
Children and adults living at home with family
Home-Based Services offers a facilitator to support families receiving services for children and adults with intellectual and developmental disabilities who are living at home.
Blue Cap has supports and services for people of all ages, and every program is designed for each individual. For a family at home, that support comes through a Service Facilitator.

What Service Facilitation includes
Service Facilitation includes case management services. Those services assist individuals and families in gaining access to needed Waiver and other Medicaid State Plan services, and to medical, social, educational and other services.
The service plan rests on the same person-centered planning behind every service at Blue Cap, where everyone receives a comprehensive assessment and customized recommendation.
Day-to-day oversight of the service plan
Service Facilitators are responsible for the day-to-day oversight and administration of the service plan, and for ensuring the health, safety and welfare of the individual.
Questions about Home-Based Services go to Carmanleatha Meeks, the Home-Based Services Coordinator. The contact details are just below.
